Ordering from the DSHB
There are several options available to order from the DSHB. Orders can be placed on line by customers using American Express, MasterCard or VISA as their method of payment. For those not ordering by credit card on line, we still accept orders by fax and by email.




Credit Card Orders over the Internet
To use this option, you must first register at the DSHB website. In the upper right corner of the webpage, click on the Register link, if you are already registered, click Log In to begin ordering.



Fax and Email
To order using one of these methods, click on the Order Forms tab to download or print the DSHB order form. Complete the order form, providing American Express, MasterCard or VISA credit card details or a purchase order number. If an original purchase order is available from your institution, we would appreciate receiving a copy of it when you send the DSHB order form. Either fax the completed paperwork to us at 319-335-2077 or scan the paperwork and send as an attachment to dshb@uiowa.edu. Please indicate on your order if you would like to receive a receipt or ship date confirmation.
